What is Care Management?

Care Management is about knowledge, experience and resources -- all combined to give the older adult specialized care services by:

• Identifying, assisting and exploring options for personalized care.
• Respecting and fostering independence for older persons and families.
• Monitoring changing health status needs that may range from home to nursing home.
• Developing a personalized care plan for each family and client with the best options for care.

Mary has been caring for my mother for a little over a year. She visits her once a week, sets up all her medications, arranges for refills as necessary and picks them up. She also monitors my mom's blood pressure and overall state of health. Beyond this, she has been an invaluables source of support for both my mom and myself. Her expertise and experience in the field of senior care is exceptional, and we feel blessed to have discovered her. (Claudia V.)

Mary helped us understand what was happening and what options were available to us. Her gentle prodding (sometimes not so gentle) caused those assisting my mother to work at a higher level and it was obvious that the doctors respected her opinions also.
